Doncaster Primary Care Networks Linking to YHCR
Primary care networks in Doncaster are among the first in the Yorkshire and Humber region to be linked up to the Yorkshire and Humber Care Record (YHCR).
The South Yorkshire Shared Care Record (SYSCR) Programme, overseeing the roll-out of the YHCR in the area, has teamed up with Primary Care Doncaster (GP federation) and Doncaster South and Doncaster North-West Primary Care Networks (PCNs) to make the shared care record platform available to more users in primary care.
The GP federation and the two PCNs went live with the YHCR this spring and following evaluation of this initial project, it is planned that more PCNs could gain access later in 2026. In total, there are also 169 GP practices being onboarded onto the YHCR.
Enrolling the GP federation and PCNs onto the shared care record platform supports the aims of the new NHS Neighbourhood Health Framework where: “The NHS will reduce bureaucracy so GPs can focus on delivering better care. ICBs will implement the ‘Red Tape Challenge’, improving the connection between primary and secondary care through a range of common-sense interventions,” including via access to shared care records.
The YHCR brings together people’s separate records into a structured format. This gives health and care professionals directly involved in an individual’s care a more complete view of the care and treatment that an individual has received across all services. The shared care record platform securely connects information from hospitals, GPs, mental health services and social care providers, ensuring professionals have the most up-to-date information when they need it.
Having access to the YHCR is set to bring time-saving benefits to primary care staff, reducing the number of times they have to sign in and out of different SystmOne units to make appointments for their patients. It will also open up access to patient information across NHS and social care providers throughout the Yorkshire and Humber region, improving the quality and safety of care.
The YHCR is being rolled out via the South Yorkshire Shared Care Record (SYSCR) Programme, part of the national Connecting Care Records initiative. The programme is linking up GP practices and NHS organisations, local authorities, and voluntary and community sector organisations including service providers and hospices in South Yorkshire, across the Barnsley, Doncaster, Rotherham and Sheffield areas.
