By partnering with Interweave, health and care organisations are providing digitally enhanced and inclusive care. Our shared real-time data powers the connectivity within our partner regions to identify unmet needs, reach unserved populations, and coordinate care more effectively and efficiently.

Our technology works seamlessly to capture and process clinical information, reducing the documentation burden on clinicians. By automating tasks like referrals and patient communications, we free up valuable time allowing clinicians to focus more on delivering high-quality, compassionate care.

From its beginnings as the Yorkshire and Humber Shared Care Record, the Interweave platform is now being utilised across multiple Integrated Care Systems, delivering benefits to hundreds of participating health and care organisations and ultimately enabling more effective patient care to almost 8 million individuals. The infographic opposite provides an overview of 2024 and we will use this page to provide updates on coverage, usage and benefits. We will also showcase the benefits identified by the Interweave Partners in their implementation programmes.

The Humber and North Yorkshire Digital Transformation Programme is delighted to be able to share with you the finalised Efficiencies report for 25/26.

The Humber and North Yorkshire Shared Care Record Annual Report provides a comprehensive overview of the key benefits realised through the implementation of our regional Shared Care Record – the Yorkshire & Humber Care Record (YHCR).

This summary includes:

  • Hours saved (including full-time equivalent estimates)
  • Reduced conveyances
  • Decrease in CO2 emissions
  • User Experience and Feedback

Why is this report important?

Capturing and showcasing system-wide benefits is essential to demonstrating the real-world impact of having the right information available at the right time. Digital solutions like the YHCR play a key role in building a health and care system that is both efficient and effective.

By evidencing these benefits, we strengthen organisational engagement, encourage clinical buy-in, and empower teams to embed the YHCR into everyday practice.

Without consistently measuring outcomes, we cannot fully understand or communicate the value that digital tools are bringing to care delivery.

South Yorkshire Shared Care Record

Yorkshire and Humber Care Record in South Yorkshire

The South Yorkshire Shared Care Record (SYSCR) programme is advancing the Yorkshire and Humber Care Record (YHCR) in the region. A total of 1.4 million people’s records in South Yorkshire are now accessible on the YHCR to professionals directly involved in their care. As part of our benefits realisation work, financial modelling of the expected impact of increasing numbers of staff using the platform reveals that efficiencies of nearly £22 million could be realised over the next five years. 

Illustrated within the SYSCR Benefits Report are stories around the frontline efficiencies SYSCR anticipate being realised over the coming years.
