Leeds Community Healthcare Data Goes Live in YHCR Following Landmark Collaboration
Leeds Community Healthcare (LCH) and Interweave are pleased to announce the successful go‑live of structured SystmOne Non‑GP data within the Yorkshire & Humber Care Record (YHCR), marking a significant milestone in regional interoperability and cross‑organisational collaboration.
Delivered in partnership with Synanetics and Interweave, this initiative provides seamless integration of LCH’s data into a range of YHCR panels, enabling clinicians to access vital information more efficiently and in near real time. This enhanced access supports improved clinical decision‑making and strengthens the quality and continuity of patient care across the region.
The newly available data includes a comprehensive set of resources – Appointments, Encounters, Episodes of Care, Flags, Locations, Medications, Organisations, Patients and Referral Requests. These are now all accessible across the live environment as of 04 March 2026.
This implementation represents a first‑of‑type achievement, setting a benchmark for future data integration efforts across the NHS. It reflects the power of collaborative working and the shared commitment of multiple organisations united by a common purpose: enabling better care through better data.
