GP, Hospital and Social Care Data Proving Very Useful to LLR Care Record Users
Interweave’s partner, the Leicester, Leicestershire and Rutland (LLR) Care Record, have conducted a survey of staff using the care records. The survey discovered what data sources their users they appreciate most – with access to the GP record and hospital discharge information featuring prominently.
In the survey, adult social care staff at Leicestershire County Council placed the greatest value on being able to access the GP record, which includes details on medication, conditions and appointments. They also appreciated being able to access hospital discharge information from Leicestershire Partnership NHS Trust (LPT) community hospitals, and information on admissions and emergency department visits from University Hospitals of Leicester NHS Trust (UHL). Seeing whether people had assistive equipment at home was also highly valued.
Staff within LPT’s community health services placed most value on seeing information about UHL appointments and LPT hospital stays, as well as adult social care services and the GP record. Access to this information is saving time, improving care decisions and enabling better coordination of care alongside other involved services.
The LLR Care Record team also used the survey to find out what sources of information not currently available would be most useful to health and care practitioners in being able to access. Requests included more information on LPT and adult social care involvements with people, UHL discharge letters and outcomes, test results and out-of-area activity.
Sandra Taylor, LLR Care Record benefits and change manager, who oversaw the survey, said: “We thank everyone who took the time to feed back to us. It was good to see what people were finding most useful in their work and also what missing information they would like to see included in the Care Record to help guide our future plans.”
A total of 20,000 staff in LLR health and care providers and East Midlands Ambulance Service have access to the LLR Care Record for direct patient care. It has been rolled out across adult social care in Leicester City Council, Leicestershire County Council and Rutland County Council, and rollout is progressing in Leicestershire Partnership Trust, University Hospitals of Leicester, Patient Care Locally CIC, GP practices, LOROS hospice and community pharmacies.
The LLR Care Record is joining up people’s health and care records across the NHS and local authorities. It means information recorded about someone’s health and care such as illnesses, medications and hospital admissions can be accessed by different people who are directly involved in their care. Hospitals, GPs and other health and care workers have always tended to have separate records – the LLR Care Record allows the data from these to be seen in one place.
