Sue Ryder Hospices Go Live in West Yorkshire

In November, the Yorkshire & Humber Care Record reached an important milestone in West Yorkshire with the successful go-live of its first two Sue Ryder hospices. Sue Ryder supports people through the most difficult times of their lives, providing expert end-of-life care and bereavement support. Manorlands Hospice and Wheatfields Hospice are now fully live on the shared care record, powered by Interweave technology, marking a significant step forward in improving information sharing across the region.

Interweave provides the underlying digital infrastructure that enables secure, real-time access to relevant patient information across health and care organisations. This technology supports more joined-up, coordinated care by ensuring clinicians and care professionals have the information they need when it matters most.

The inclusion of Manorlands and Wheatfields reflects the growing role of hospices within integrated care systems and strengthens collaboration between Sue Ryder and partner organisations across West Yorkshire. This first phase also lays the groundwork for further Sue Ryder services to join the Yorkshire & Humber Care Record in the future, helping to enhance continuity of care and improve outcomes for patients and families.
