Social Enterprise Organisation Becomes Latest to Benefit From the Shared Care Record

Interweave are pleased to share that a new social enterprise has joined the Yorkshire and Humber Care Record (YHCR). The health services provider FCMS has become the latest organisation to roll-out the YHCR to its clinicians and managers.

FCMS is a social enterprise health and wellbeing services provider, running a range of services across the country, including South Yorkshire. Founded in 1994 as a GP cooperative providing home visiting services across Blackpool, Fylde and Wyre, FCMS has grown into a multi-service health and wellbeing services provider. Its services include urgent care, diagnostic services, emergency dental services, call handling, and targeted care for vulnerable communities.

FCMS joins 13 other local NHS organisations and local authorities, as well as all GP practices in adopting the YHCR. The rollout is taking place through the South Yorkshire Shared Care Record (SYSCR) Programme, part of the national Connecting Care Records initiative, covering the Barnsley, Doncaster, Rotherham and Sheffield areas.

The YHCR brings together people’s separate records into a structured format. This gives health and care professionals directly involved in an individual’s care a more complete view of the care and treatment that an individual has received across all services. The shared care record platform securely connects records from hospitals, GPs, mental health services and social care providers, ensuring professionals have the most up-to-date information when they need it.

Nicki Sherburn, FCMS Clinical Manager, Doncaster, said: “The ability for our clinicians to access the YHCR will be a positive enhancement to the care we deliver. Being able to view real-time health and care information, across care providers and between different systems, supports decision making and allows us to deliver joined up, holistic, safer care.
