At Interweave we are pleased to announce the joining of NHS Dorset Integrated Care Board (ICB) to the Interweave Partnership with their Dorset Care Record.

Joining our portfolio of six other NHS regions and adding over 800,000 citizens, the new partnership brings the total population covered to over 10 million people. This is a significant milestone in Interweave’s progress towards their mission to improve joined-up care across the country.

The Dorset Care Record is a county-wide partnership of health and social care organisations, including hospitals and GPs. For the people who live locally, it’s a safe way to keep all of their electronic health and social care information in one place, making it quicker and easier for the appropriate health and social care professionals, across a variety of services, to view information, make safe decisions, and provide the right care, at the right time.

Ian Clucas, Deputy CIO at Interweave, said, “We’re thrilled to formally introduce Dorset to the Interweave Partnership. It has been a positive experience working with our colleagues in Dorset over the past months, who share our vision of providing digitally enhanced and inclusive care.

Thank you to everyone who has worked hard on this project, enabling us to reach this significant milestone. We very much look forward to continuing and building on this progress together as we move forwards.”

Interweave has been uniquely designed to utilise modern technologies and is delivered by our in-house team, which is a part of Humber Teaching NHS Foundation Trust. With support from our supply chain partners, the code used to develop Interweave is open-source and the Intellectual Property is owned by the NHS.

By working together with several Integrated Care Systems (ICS) across the NHS landscape nationally, the Interweave Partnership enables better care coordination to improve the health and social care outcomes for people across the country.

The Interweave Partnership has recently been shortlisted in the Health Tech Newspaper (HTN) Awards 2026 for ‘Partnership of the Year’. Recognising the strength of the partnership and the progress made to date, to improve health and social care experiences across various populations and communities.

Similarly, Interweave also won the HSJ Digital Award for ‘Driving Interoperability and Standards Through Digital’ at the 2026 ceremony.

We’re delighted to announce that we, alongside all our partners across the Interweave network, have been shortlisted for the Health Tech Newspaper (HTN) Award for ‘Partnership of the Year.’

The HTN Awards 2026 celebrate digital teams, programmes, innovations and suppliers which have made a difference throughout the year, helping to shape future services and systems across health and care.

This shortlist acknowledges ongoing work to scale a sustainable, NHS-owned Shared Care Record ecosystem which connects almost 10 million people. The award is fantastic recognition of the commitment of all Interweave partners, who work together to provide high quality, joined-up care for our local communities.

Interweave is a data platform that powers several Shared Care Records across the country, including:

Notts Care Record – Nottingham and Nottinghamshire Integrated Care Board (ICB)

Leicester, Leicestershire and Rutland (LLR) Care Record – Leicester, Leicestershire and Rutland ICB

Yorkshire and Humber Care Record (YHCR) – Humber and North Yorkshire Health and Care Partnership, South Yorkshire Integrated Care System (ICS), and West Yorkshire ICB

Dorset Care Record – NHS Dorset ICB

Joined Up Care Derbyshire – Derby and Derbyshire ICB

These Shared Care Records powered by the Interweave platform, enable the secure sharing of clinical information across organisations and care settings. By bringing information together in one place, Interweave supports health and care professionals to access relevant information when needed, helping to inform decision-making and reduce duplication. For service users, this contributes to a more joined-up experience of care throughout their healthcare journey at the point of need.

Lee Rickles, CIO at Humber Teaching NHS Foundation Trust, said:

“We’re thrilled to be shortlisted for the Partnership of the Year category at this year’s HTN Awards. This recognition is testament to the ongoing dedication of all our partners, who work together to improve the Shared Care Records landscape and improve outcomes for service users and communities across the country.

Congratulations to all my colleagues who are shortlisted as part of this award.”

This year’s HTN Awards Ceremony will take place online from 12pm on 7 October 2026.

Congratulations to our fellow shortlisted organisations. You can find the full list and more about the Partnership of the Year category on the HTN website.

Good luck to all organisations nominated across the categories at this year’s awards.

Interweave is proud to share this recognition across our partner network as we continue working together to support joined-up care across the country.
We’re happy to announce that new integration now gives authorised health and care professionals access to NHS 111 clinical information from Yorkshire Ambulance Service (YAS), through the Yorkshire and Humber Care Record (YHCR), powered by Interweave.

The availability of this data will help health and care professionals gain a richer understanding of a patient’s urgent care journey, reducing information gaps, supporting faster clinical decisions and helping to improve continuity of care across organisational boundaries. This marks a significant milestone in improving information sharing across the region.

Delivered collaboratively by YAS, Synanetics and the YHCR team, the project has integrated NHS 111 data from the YAS Computer Aided Dispatch (CAD) system into the Interweave architecture.

This partnership was established to enhance integrated care, ensuring that information collected during NHS 111 interactions can be securely accessed by authorised professionals involved in a patient’s care. The programme has focused on interoperability, clinical safety, information governance and user accessibility throughout its delivery.

What is the benefit of the new data?

The new data provision enables authorised users, including GPs, emergency departments, ambulance services and mental health practitioners, to securely access NHS 111 information directly through YHCR, providing a more complete view of a patient’s recent interactions with urgent care services.

Through the new service, authorised clinicians can access key information including:

Patient demographics

Presenting complaints and symptoms

Clinical assessment summaries

NHS 111 triage outcomes and dispositions

Referrals to other services

Advice provided to patients

Escalations and follow-up actions.

Ian Clucas, Deputy CIO at Interweave, said: “The inclusion of NHS 111 clinical data within the Yorkshire and Humber Care Record is an important step in our mission to provide health and care professionals with the information they need, when they need it.

By making urgent care information available through the YHCR, we’re helping clinicians build a more complete picture of a patient’s needs, supporting safer, more coordinated and effective care across the region.”

Ola Zahran, Chief Technology Officer at Yorkshire Ambulance Service NHS Trust, said: “With enriched patient information now being available to ambulance clinicians to support assessment and treatment, this means that patients will benefit from more joined-up, safer care.”

How was the integration developed?

The integration has been designed using nationally recognised Fast Healthcare Interoperability Resources (FHIR) standards and operates within robust NHS information governance frameworks, ensuring secure access, role-based permissions and comprehensive audit controls.

Looking ahead

The successful go-live reflects the continued commitment of the partnership to improve data sharing and enable joined-up care for citizens across Yorkshire and the Humber.

As the YHCR continues to expand the range of information available through its shared care record, the addition of NHS 111 clinical data represents a major advancement in providing healthcare professionals with timely access to critical information that supports positive patient outcomes.

Interweave is proud to share these milestones across our partner network as we work together to join up care nationally.
A new landmark has been achieved for the Yorkshire and Humber Care Record (YHCR) in South Yorkshire – with the biggest ever monthly total of records being accessed.

In July 2026, 31,772 records were accessed – the first time the South Yorkshire Shared Care Record (SYSCR) programme has broken through the 30,000 landmark.

There were particularly high levels of access to the shared care record platform within Doncaster and Bassetlaw Teaching Hospitals NHS Foundation Trust, Sheffield Teaching Hospitals NHS Foundation Trust and Barnsley Hospital NHS Foundation Trust.

The number of records being accessed for July was a 17 per cent increase on the figure for the previous month.

Katie Dowson, South Yorkshire Shared Care Record (SYSCR) Programme Director, said: “The continual increase in the numbers of health and care staff accessing the YHCR is pleasing and demonstrates how more and more individuals and teams are realising the benefits of being able to view a more detailed picture of the person they are caring for. We are keen to see this positive trend continue, with greater utilisation of the YHCR helping to underpin the delivery of high-quality, integrated care across South Yorkshire.”

The YHCR gives health and care professionals directly involved in an individual’s care a more complete view of the care and treatment that an individual has received across all services. The shared care record platform securely connects information from hospitals, GPs, mental health services and social care providers, ensuring professionals have the most up-to-date information when they need it.

Primary care networks in Doncaster are among the first in the Yorkshire and Humber region to be linked up to the Yorkshire and Humber Care Record (YHCR).

The South Yorkshire Shared Care Record (SYSCR) Programme, overseeing the roll-out of the YHCR in the area, has teamed up with Primary Care Doncaster (GP federation) and Doncaster South and Doncaster North-West Primary Care Networks (PCNs) to make the shared care record platform available to more users in primary care.

The GP federation and the two PCNs went live with the YHCR this spring and following evaluation of this initial project, it is planned that more PCNs could gain access later in 2026. In total, there are also 169 GP practices being onboarded onto the YHCR.

Enrolling the GP federation and PCNs onto the shared care record platform supports the aims of the new NHS Neighbourhood Health Framework where: “The NHS will reduce bureaucracy so GPs can focus on delivering better care. ICBs will implement the ‘Red Tape Challenge’, improving the connection between primary and secondary care through a range of common-sense interventions,” including via access to shared care records.

The YHCR brings together people’s separate records into a structured format. This gives health and care professionals directly involved in an individual’s care a more complete view of the care and treatment that an individual has received across all services. The shared care record platform securely connects information from hospitals, GPs, mental health services and social care providers, ensuring professionals have the most up-to-date information when they need it.

Having access to the YHCR is set to bring time-saving benefits to primary care staff, reducing the number of times they have to sign in and out of different SystmOne units to make appointments for their patients. It will also open up access to patient information across NHS and social care providers throughout the Yorkshire and Humber region, improving the quality and safety of care.

The YHCR is being rolled out via the South Yorkshire Shared Care Record (SYSCR) Programme, part of the national Connecting Care Records initiative. The programme is linking up GP practices and NHS organisations, local authorities, and voluntary and community sector organisations including service providers and hospices in South Yorkshire, across the Barnsley, Doncaster, Rotherham and Sheffield areas.

Developed in collaboration with Care Networx and commissioned by the NHS West Yorkshire Integrated Care Board (ICB), Share2Care provides unpaid carers with a secure platform to coordinate caring responsibilities, collaborate with family and friends, and create digital contingency plans that can be shared with healthcare professionals.

The programme has the potential to support up to 250,000 unpaid carers across West Yorkshire and could therefore serve as a model for other regions seeking to strengthen support for carers and improve connections between health and social care services.

This represents an important step forward in recognising and supporting the vital role of unpaid carers, by ensuring that crucial information can be made available across health and care settings, supporting informed decision-making and the continuity of care.

Supporting Better Information Sharing

Every day, unpaid carers provide essential support to family members, friends and loved ones, often coordinating complex care arrangements behind the scenes. Yet much of that information has traditionally remained outside formal health and care records.

Through Share2Care, carers can create structured contingency plans that outline what should happen if they are suddenly unable to provide care. The new solution means that care plans are integrated with NHS systems, enabling authorised healthcare professionals to identify plans and access relevant information when required.

For the YHCR, this programme reflects our shared ambition to ensure that important information follows the individual across care settings and that the contributions of unpaid carers are better recognised within the wider health and care ecosystem.

Empowering Carers Through Digital Innovation

The Share2Care platform provides practical tools that help carers manage day-to-day responsibilities. Through a trusted “circle of care”, users can coordinate support with family members, friends and others involved in caring. They can also share updates, communicate securely and manage administrative tasks from a single platform.

Alexis Twigg, Founder of Care Networx, said: “Care has always happened in the community, led by families and unpaid carers, but the system has never truly seen it.

Share2Care changes that by enabling carers to record and share information in a way that can support better decisions across the health and care system.”

This supports the NHS Long Term Plan commitment to improve contingency planning for unpaid carers, helping ensure more carers can benefit from structured planning conversations and that their plans become accessible to healthcare professionals when needed.

Strengthening Integrated Care Across Yorkshire and Humber

As one of the UK’s largest shared care record programmes, the YHCR, powered by Interweave, is focused on enabling secure and appropriate information sharing across organisations to support safer, more coordinated care.

By helping carers record information digitally and making key contingency arrangements, Share2Care demonstrates how digital innovation can bridge the gap between community-based care and formal healthcare services.

Kulvant Sandhu, Senior Programme Manager for Unpaid Carers at NHS West Yorkshire ICB, said: “We are delighted to have commissioned Share2Care, an innovative digital solution that helps carers coordinate care more effectively by ensuring important information is readily available, including carer identification and emergency contingency arrangements.

By supporting carers in their day-to-day role, it offers greater peace of mind and reassurance that arrangements are in place when they are needed most.”

The East Riding HealthiER Referrals programme has become the first social prescribing partner to go live with the Yorkshire & Humber Care Record (YHCR), powered by Interweave, marking another important milestone in extending connected care beyond traditional NHS settings.

HealthiER Referrals supports people referred by their GP into local leisure and wellbeing services, helping residents improve their physical and mental health through tailored activity and lifestyle support.

By providing authorised staff with secure access to the YHCR, professionals now have a more complete understanding of an individual’s medical history, long-term conditions and medications, enabling them to deliver safer, more personalised support.

This launch demonstrates how shared care records can support the wider health and care system, extending the benefits of digital transformation into community-based services.

It also aligns with the ambitions of the NHS 10 Year Health Plan by:

Strengthening prevention through effective social prescribing

Supporting care closer to home within local communities

Enabling professionals across organisational boundaries to work from the same trusted information

Improving patient safety and reducing the need for people to repeatedly share their medical history

This is a great example of digital transformation supporting prevention in practice – using connected records to enable safer, more informed community care and demonstrating how integrated working can help deliver the ambitions of the NHS 10 Year Health Plan.

Twelve months after its launch, the Notts Care Record (NCR) is playing an increasingly important role in helping health and care professionals across Nottingham and Nottinghamshire access the information they need to provide safe, joined-up care. All powered by Interweave technology.

Over the past year, the Notts Care Record has grown significantly, bringing together information from NHS, local authority and care partners into a single shared view.

The result is quicker access to vital information, fewer delays, less duplication and better-informed decisions for the people and communities we serve.

Significant growth across the system

Since launching in July 2025, more than 12,300 health and care professionals have accessed the Notts Care Record, with a total of 721,327 records viewed across the system. Growth has been consistent throughout the year. In its first month, the NCR was used by 1,274 unique users, with 14,382 records accessed. By the end of June 2026, monthly usage had increased to 6,545 unique users accessing 97,177 records – demonstrating growing confidence in the system and its value to frontline teams.

The number of organisations connected to the record has also increased. In July 2025, three partners had rolled the record out to their teams. Twelve months later, ten partners are using the NCR to access information, helping to create a richer, more complete picture of an individual’s health and care journey.

Making a difference to users

While the numbers highlight the scale of adoption, feedback from colleagues across the system demonstrates the difference the Notts Care Record is making in practice. Teams within Nottinghamshire County Council’s Contact and Advice Service report that access to the NCR has helped them make faster decisions, ensure people are directed to the right support first time.

It has reduced duplication across organisations by providing visibility of activity already taking place elsewhere in the system. Clinical teams have also described benefits in preparing for appointments and visits, accessing previously unavailable information and improving conversations with patients and service users.

Whilst colleagues supporting people at the end of life have highlighted how shared access to information can reduce the need for individuals and families to repeat their story. Hospital and community teams have welcomed the ability to access a broader range of information when assessing and supporting patients.

Feedback from partners also emphasises the ease of use of the platform. Clinicians have praised the breadth of information available through the record and the ability to access key information quickly when working in busy environments.

‘999’ normally means an emergency for the health and care system – but in the case of the Yorkshire and Humber Care Record (YHCR) in South Yorkshire those numbers are very welcome.

On 18 June 2026, the South Yorkshire Shared Care Record (SYSCR) programme recorded 999 unique patient records being accessed – a daily record for the region.

Of those 999 unique patient records, the highest Trusts using the shared care record platform were Sheffield Teaching Hospitals NHS Foundation Trust, Barnsley Hospital NHS Foundation Trust and Doncaster and Bassetlaw Teaching Hospitals NHS Foundation Trust.

On the same day, there were 1,303 total patient records accessed and 1,324 total logins. The numbers show a fairly steady month-on-month growth in the number of people using the YHCR.

Katie Dowson, South Yorkshire Shared Care Record (SYSCR) Programme Director, said: “We are very pleased to see the growth in people using the shared care record platform. More and more colleagues are seeing the benefits in being able to view more information than ever before about the person they caring for. We would encourage everyone to login and see the amount of information the YHCR now contains.”

A new Carer Flag has been introduced within the Yorkshire & Humber Care Record (YHCR) for West Yorkshire.

This will indicate when a patient is an unpaid carer for someone else, who may need additional help if the carer is admitted to, or conveyed to, hospital.

An unpaid carer is anyone who provides unpaid care or support to a family member or friend who has a disability, illness, or who need support in later life due to frailty, and they could not manage without that care or support.

This is an incredibly important piece of work as it helps identify when unpaid carers are unexpectedly taken into hospital or another health service. Having clear, visible information about this helps clinicians act quickly to ensure contingency plans are in place for the cared-for person.

This is a regional initiative to strengthen support for unpaid carers and goes hand-in-hand with enhanced communications to help staff on the ground understand what the Carer Flag looks like in the shared care record, and what to do when they encounter this marker when working with digital tools.

We’re proud of our shared care record teams across West Yorkshire, South Yorkshire and Humber and North Yorkshire and look forward to continuing to work together to implement innovative solutions that strengthen patient safety and user experience.

To learn more about what we do nationally to support shared care records, please consider subscribing to the Interweave quarterly newsletter.
