East Riding HealthiER Referrals becomes the first social prescribing service to access the YHCR through Interweave
The East Riding HealthiER Referrals programme has become the first social prescribing partner to go live with the Yorkshire & Humber Care Record (YHCR), powered by Interweave, marking another important milestone in extending connected care beyond traditional NHS settings.
HealthiER Referrals supports people referred by their GP into local leisure and wellbeing services, helping residents improve their physical and mental health through tailored activity and lifestyle support.
By providing authorised staff with secure access to the YHCR, professionals now have a more complete understanding of an individual’s medical history, long-term conditions and medications, enabling them to deliver safer, more personalised support.
This launch demonstrates how shared care records can support the wider health and care system, extending the benefits of digital transformation into community-based services.
It also aligns with the ambitions of the NHS 10 Year Health Plan by:
- Strengthening prevention through effective social prescribing
- Supporting care closer to home within local communities
