Our Yorkshire & Humber Care Record (YHCR) has been awarded Highly Commended in the Digital Clinical Safety Award category at the prestigious Health Service Journal (HSJ) Digital Awards 2024.

The 2nd Annual Shared Care Record Summit brought together hundreds of Health and Social Care professionals to showcase national, regional, and local progress in sharing information through a healthcare setting. The event ran over 16 & 17 April and was held at the ICC in Birmingham.

We are delighted to announce that we have been shortlisted for a Health Service Journal (HSJ) Digital Award.

This nomination comes as welcome recognition of the hard work achieved by Humber Teaching NHS Foundation Trust’s Interweave team and their partners, who are involved in the integration of shared care records across Yorkshire and the Humber and beyond.

Calderdale and Huddersfield NHS Foundation Trust (CHFT) are advancing the sharing of their patient data through data maturity to support our regional shared care record. This will deliver more joined up patient information to support the provision of excellent clinical and social care and close data gaps across West Yorkshire.

The Yorkshire & Humber Care Record (YHCR) allows clinicians to see information from multiple organisations about patients and use it for direct clinical care. This is a significant step forward in both increasing patient safety and informed decision making.

Powered by Interweave, progress is being made in bringing pharmacies into the Leicester, Leicestershire and Rutland (LLR) Care Record – further broadening the scope of the health and care programme and the potential benefits for local people.

The LLR Care Record is bringing together people’s separate health and care records into a structured, easy-to-read format all in one secure place. It joins up information based on the individual rather than one organisation.

Following on from the success of the first Shared Care Record Summit held in Leeds in March 2023, we are delighted that our next annual summit will be at the ICC in Birmingham, 16-17 April 2024.

We’re thrilled to receive some great feedback from users during the early stages of implementation at Bradford Council.

Yorkshire Ambulance Service NHS Trust (YAS) is now sharing the electronic care record for emergency ambulance frontline and call centre patient encounters through the Yorkshire & Humber Care Record (YHCR), providing a rich history to help inform ongoing patient care.

Together with Leeds Teaching Hospitals NHS Trust and West Yorkshire Integrated Care Board (ICB), we will be discussing the benefits of Shared Care Records at the Leeds Digital Festival on 27 September at 11am. This is a virtual event and the session can be accessed via the Read Post button below.
