Adult Social Care Services Data Added in South Yorkshire

The Yorkshire and Humber Care Record (YHCR) has become even more useful to health and care practitioners with the addition of more organisational data on the care of patients/citizens.

In the past month, the YHCR has added adult social care services data from Barnsley Metropolitan Borough Council and ‘related persons’ data such as family members/carers from Sheffield City Council, all helping to make the shared care record ever more comprehensive.

The YHCR gives health and care professionals directly involved in an individual’s care a more complete view of the care and treatment that an individual has received across all services. The shared care record platform securely connects information from hospitals, GPs, mental health services and social care providers, ensuring professionals have the most up-to-date information when they need it.

Katie Dowson, South Yorkshire Shared Care Record (SYSCR) Programme Director, said: “The more information that health and care professionals have about an individual, the better care and support they can provide. This will result in not only improvements in the quality of care but will also help save time for busy professionals in seeking out the information they need.

“The information being added in recently from Barnsley and Sheffield further strengthens the shared care record platform and we would like to thank all those working with us in the local authorities to make this happen.”

The YHCR is being rolled out via the South Yorkshire Shared Care Record (SYSCR) Programme, part of the national Connecting Care Records initiative. The programme is linking up GP practices and NHS organisations, local authorities, and voluntary and community sector organisations including service providers and hospices in South Yorkshire, across the Barnsley, Doncaster, Rotherham and Sheffield areas.
