Information Governance
This application form should be completed by any third-party organisation looking to onboard to Interweave Products. A third-party organisation is any organisation that is not an NHS Trust or Local Authority:
Each organisation will be required to sign a Data Protection and Information Sharing Agreement, examples of these documents are below:
Some basic organisation information is required in order to draft the above agreements which can be submitted by filling out the MS Form relevant to your ICB below:
Help is also available for third party organisations when completing a Data Protection Impact Assessment. The above links have the option to answer up to nine more questions which will generate a template DPIA for your organisation.

Clinical Safety
A Clinical Safety Officer will be required from the organisation to oversee the clinical risk assessment. They are expected to be involved in the risk assessment work and sign-off the documentation.

Data Assurance
